Home Health Certification
Path: Physician Certification of Care Plan <
When the physician signs the Plan of Care (HCFA – 485), he/she certifies that:
  • The patient is homebound.
  • The patient needs a qualified skilled service/
  • The Plan of Care is established and reviewed by the physician.
  • The services were provided while the patient was under the care of a physician.

The certification must be signed by a qualified physician:

  • Doctor of Medicine
  • Doctor of Osteopathy
  • Doctor of Podiatry for services within his/her scope of practice.
