The Home Care and Hospice Foundation is dedicated to enriching quality of life for patients of the Members of JHS  whose independence and dignity can best be preserved, and whose families may be kept together, by receiving health care services and products in the home. The Home Care and Hospice Foundation is further dedicated to the pursuit of excellence as a leader in assuring access to home care services. By embracing leading-edge issues, it aims to assist The Home Care Network in becoming the recognized and preferred home care resource for residents of communities served by the Members of Jefferson Health System.
