Our clinical programs provide specialized care for the following:
- Cardiac - for patients with congestive heart failure or recovering from heart surgery.
- Diabetes - for patients newly diagnosed with diabetes or new to insulin therapy.
- Maternal-Child - for patients with gestational diabetes or other complications of pregnancy, and for well-mom/baby visits, including lactation consultation.
- Medical - for patients diagnosed with new or chronic medical conditions, such as stroke or pulmonary disease.
- Oncology - for patients whose primary diagnosis is cancer.
- Orthopedics - for patients recovering from a fracture or recovering after hip or knee replacement surgery, or after neurosurgery.
- Ostomy and Continence - for patients with an ostomy, a fistula, or with incontinence.
- Surgical - for patients recovering from a surgical procedure.
- Telemonitoring - remote patient monitoring for those with chronic illness.
- Wounds - for patients with wounds that are not healing properly.
